Primary care is under pressure due to, among other things, an increase in chronic diseases and a decrease in healthcare personnel. In addition, a portion of the requests for help to the GP concern problems that might be better resolved by another professional. This applies, among others, to adult patients with (pre-)diabetes type 2. Referral via a group consultation from the GP practice to appropriate programs, such as a combined lifestyle intervention (GLI), could potentially be a suitable innovation to reduce pressure on the GP practice and guide patients to the right care in the right place by the right professional. A GP practice in Amsterdam North wants to investigate the implementation of a group consultation to refer patients to the GLI ‘Keer Diabetes Om – Voeding leeft’.
